If you meet the criteria and would like to apply you can either: 

  1. Call the NDIS directly to make an access request on 1800 800 110.  
  2. Fill out the Access Request Form and submit it via email to NAT@ndis.gov.au  
  3. Contact your local LAC or NDIA office. 

 To apply for the NDIS, you will need to provide:  

  1. A completed Access Request Form.  
  2. Information about your age, residency and disability or need for early intervention. This is known as supporting information.  

Once you have successfully completed your Access Request Form and sent in all of the correct supporting information, it can take up to 21 days to hear back from the NDIS.  

Every person who is a participant of the NDIS will then be able to create a plan. This is a document that is developed by you and the NDIA planner or your local area coordinator.
