Once you have reviewed whether you meet the requirements to apply for the NDIS, you can apply in one of two ways:
- You can make a verbal request over the phone by calling them directly on 1800 800 110, or
- Fill out the Access Request Form and send it back to them via email at NAT@ndis.gov.au
You can download the Access Request Form here or you can ask them to send you the form via mail by calling them directly on 1800 800 100.
Your doctor or treating health professional will need to complete section 2 of the form or, if you have existing reports or assessments, you can send them in with your completed form.
